NEW APPLICATION FORM FOR PART D MASC
Here is a message that I just received from Mike Yacentiuk, Senior Manager of MAFRI. This was in response to a letter I sent him expressing my concerns with all the changes and revisions that are taking place which are resulting in even MORE application forms.
After receiving this Mike has asked me to meet him on Monday Sept 19th to discuss ISSUES. I will keep you posted.

Most individuals have already filled out a Part C application form. For those that have not, we will send them a Part C application form to sign as we encounter them.
All individuals registered under Part C will be receiving a letter shortly. A Part D application will be included in the letter for those wishing to apply for Part D financial assistance.
